About West Lodge

West Lodge is for people living with complex mental health needs and require a minimum of 2 hours 1:1 support each day.

West Lodge has a team of experienced support workers to offer the 1:1 support using PBS techniques as well as a person centred approach to care. Individuals are encouraged to work towards goals and work with the support staff to develop their own support plan.

Accommodation

West Lodge is for people who require support and oversight throughout the day and night.

We can support up to 19 people at West Lodge, and each individual will require a minimum of 2 hours of 1:1 support per day.

An assessment of need will be carried out and the level of support agreed.

1:1 Support Includes

Every resident's support is tailored to them. Our team works alongside each individual across all of the following:

Managed medication and engagement with mental health treatment programmes including guidance, ordering and collecting prescriptions, teaching safe storage skills, re-ablement, and medication meetings with professionals

Develop or regain independent living skills e.g. cooking, shopping and household tasks

Identify and apply for suitable move-on accommodation (if the lodges are not suitable)

Build motivation, confidence, and self-esteem

Manage personal finances including claiming benefits, budget planning, paying bills and developing money management skills

Working with FST to manage finances in the best interest of the service user

Access education, training, volunteering, or employment

Live a full and active life in the local community

Run support groups/meetings weekly for each person to socialise

Personal care

Additional oversight for protection and safety

Emergency hospital treatment/support planning

Probation support

Access to addiction services

Support with physical health/guidance and support

Referrals

Referrals are taken via Sunderland Adult Mental Health Team.
